Welcome to Allergy and Rheumatology Clinic  of Kansas City. We look forward to getting to know you while assisting you in disease management and improving your health. There are several forms that we need to have our new patients fill out before their first appointment. In order to make sure that you have plenty of time to complete them, and to provide the best possible legibility, we have made these forms available here for you to download and complete.

After you have completed the form, please  print the forms  and get them along with you for the appointment  or you can as well email them to contact@arc-kc.org.
